Anomalous Circumflex Coronary Artery from Pulmonary Artery

The report of anomalous circumflex coronary artery from pulmonary artery (ACCAPA) was interesting.1 The authors suggested coronary angiography as the diagnostic tool, with left anterior oblique view as the best aspect for demonstrating the exact site of the anomalous connection. They performed a left coronary artery injection. We would like to point out that selective injection of right coronary artery (RCA) can also delineate ACCAPA, as was seen in a 4-month old baby.2 In figure 1
, right Amplatz catheter hooked a dominant RCA. Later part of this injection showed that RCA filled ACCAPA by collaterals. The large first branch from RCA was left anterior descending (LAD) artery with its septal perforators. Ostium of ACCAPA was seen in the region of pulmonary artery, away from the aortic sinus. Few reports of ACCAPA are available.1,2 Our patient was unique with an anomalous LAD arising from ACCAPA.
