Omental Wrap for Aortic Suture Lines

I read with interest article from Dr. Kuniyoshi et al1 and I must confess I did use above technique a few times with no regret. However I am little sceptical about using it for Isolated Ascending Aortic Suture Lines. Reasons are possible contamination of abdominal cavity and small but real risk of Myocardial Compression due to Bulky Omental Tissue occupying pericardial and/or anterior Mediastinal Space. Above technique works best for combined thoraco-abdominal or lower thoracic suture lines and can be used as an insurance policy for aorto-viscus fistula2. Again I must congratulate Dr. Kuniyoshi and his team for this important contribution in Cardio-Vascular Surgery.
