Tetralogy of Fallot with double aortic arch is exceptionally rare with a few reported cases mostly in the pediatric age group. A 27-year-old woman presented with cyanosis and exertional dyspnea, but no dysphagia or stridor. Echocardiography showed a large subaortic ventricular septal defect and overriding of the aorta with severe pulmonary stenosis. Cardiac catheterization and Angiography confirmed tetralogy of Fallot with infundibular and pulmonary valvular stenosis. Aortic root angiography showed a double aortic arch with a right-sided descending aorta; both arches were patent. The patient underwent successful repair.
