A 51-year-old man with angina pectoris was found to have an asymptomatic aneurysm of the sinus of Valsalva when contrast-enhanced multi-row detector computed tomography was performed to evaluate his coronary arteries (Figure 1Go). He rejected an operation. Four months later, he was admitted with heart failure. Transthoracic echocardiography showed rupture of the sinus of Valsalva aneurysm, and an operation was performed (Figure 2Go).

Figure 2. Intraoperative photographs of the sinus of Valsalva aneurysm. (a) After a right atriotomy, the fistula (arrow) of the aneurysm from the coronary cusp to the right atrium was seen. (b) View from the aorta. The arrow indicates the ostium of the sinus of Valsalva aneurysm.
