A 56-year-old woman was referred with an incidental finding of an intrathoracic mass on a routine check-up. Plain chest radiography and computed tomography revealed a huge cystic mass in the left hemithorax, with dense pericystic calcification that caused restriction of the lesion (Figures 1
and 2
). Bronchogenic cyst, teratoma, pericardiyal cyst, pulmonary sarcoma, or hydatid disease of the lung were the preliminary diagnoses. An exploratory thoracotomy was considered for both diagnosis and treatment, but the patient refused any intervention.

Figure 2. Thoracic computed tomography showing a cystic mass, 18 x 14 x 8 cm in size, close to the heart and extending to the diaphragm. The mass was completely restricted by a thick calcified capsula. Note the lower portion of the mass, displaying heterogenous calcification.
